Missed Appointments
A $35 fee will be charged for missed appointments or no-shows.
Late Arrivals
If you are more than 10 minutes late, the appointment will be canceled and will need to be rescheduled.
Cancellations
Please cancel or reschedule at least 24 hours before your scheduled appointment time.
FMLA & Disability Paperwork
Completion of FMLA or disability paperwork begins at $50. Payment is required before service is provided.
Completion of forms does not guarantee certification of leave, disability, accommodations, or other benefits.
Telehealth at Click Forward
Click Forward Behavioral Health utilizes in-person and telehealth services. Your telehealth appointments are designed to provide convenient, private psychiatric care while maintaining a personal connection with your provider.
✓ Join from a private, safe location whenever possible.
✓ Use a reliable internet connection and a device with working camera and audio.
✓ Do not participate while driving.
✓ Be prepared to provide your current physical location if requested.
✓ You must be in a state where your provider is authorized to practice.
✓ Telehealth may not be appropriate for every clinical situation.

Use a private space where you feel comfortable speaking openly with your provider.
